2013 designation options

  • Unrestricted gifts made to the Greatest Needs Fund will be used to fulfill our network’s future greatest employee and patient needs.
  • Help employees experiencing financial hardship by designating your donation to the Employee HELP fund.
  • Support patient and department needs by designating your gift to any of these funds:

Temporarily restricted funds

Caregivers
Provide support for scholarships for healthcare students, continuing education for Community employees, financial assistance for Community employees in need of help, and other opportunities to support Community’s caregivers

Communities We Serve
Funds Central Indiana community outreach programs and projects that support our mission to enhance health and well-being, fight disease and prevent illness, and bring better health to more people

Patients
To fund opportunities to save lives, treat the sick, celebrate moments of wellness, and to never stop caring, even when a healthcare journey comes to an end

Allen Hicks Fund
Fund new and innovative programs in the network

Ambulatory Services
Supports the ambulatory services of Community Health Network, Inc.

Angel Wings Fund
To defray some of the funeral and burial costs for our families that have experienced a stillbirth or newborn loss

Behavioral Health Services/Youth Services
Support of behavioral health youth services programs - direct patient care needs, staff support and development - (youth supplies, toys, games, books, videos, training, etc.)

Behavioral Health Services/Day Therapy
To support the patient needs of behavioral health Day Therapy Program

Behavioral Health Services/Gallahue
Support of behavioral health Gallahue programs - direct patient care needs, staff support and development

Breast Health Navigators/Specialists
To support the Breast Health Specialist program at Community Health Network

Bridges to Health/Journey to Health
To support the weight loss/lifestyle programs of Bridges to Health (Journey to Health)

Cancer/Oncology
Staff education and training, purchase of equipment for the care or comfort of cancer patients and/or their families. Facility enhancement for the care or comfort of patient or family at Community Regional Cancer Care

Case Managers Resources Fund
To support the Case Management program (including education and retention activities)

Celebration of Life
To support Celebration of Life program, a philanthropic partnership of Community Home Health Services and Community case managers, for necessary items for newborn care provided to needy families of infants born at Community

CHH Youth Bereavement Services Fund
Supports CHH youth bereavement counseling programs, including Camp Erin

Community Heart and Vascular Hospital (CHVH)
To support the needs of cardiovascular services (http://heart.ecommunity.com)

Community Heart & Vascular Physicians
Physician office staff education

Community Home Health
To support the needs of Community Home Health

Community Hospital East
Contributions restricted for use by Community Hospital East

Community Hospital East - Cardio Pulmonary Rehab
To fund unbudgeted projects (both operating and capital) in Community Hospital East's Cardiopulmonary Rehab department

Community Hospital North
Patient care and patient experience at Community Hospital North. Staff functions as it relates to ability to deliver patient care.

Community Hospital North - Ultrasound/Medical Imaging
To provide minor equipment, amenities and miscellaneous needs so the imaging patient has an exceptional experience that provides a less stressful environment and overall better customer service (Community Hospital North)

Community Hospital South
Use determined by Community Hospital South leadership.

Comprehensive Care
Items not covered in budget to support unit needs to benefit patients

Critical/Intensive Care
Nursing staff education and satisfaction

Dale Fleming Employee Emergency Assistance
To support Community employees by providing emergency financial support for terminal medical situations.

Diabetes
Used for purposes that benefit patients/staff/needed resources for diabetes care

Diversity
To support diversity of employees across the network

Education Fund
To support educational opportunities in departments that do not have designated monies assigned for continuing education

Emergency Department
To support staff education and ongoing operations of Community Hospitals East, North and South emergency departments, including equipment.

Employee Assistance (HELP) Fund
To support emergency needs of employees as a loan program that is ongoing

Family Medicine Residency General Fund
Cover education program expenses and purchase capital equipment for family medicine residency use

Health Promotion/Wellness
To support health and wellness programs for the Health Promotions department of Community Health Network

Healthy Hearts Grants
Nursing education, e.g., seminars, national meetings (Healthy Hearts Center)

Heart Failure Resources Fund
Provides resources in support of the Community Health Network Heart Failure Program

Hook Athletics
Provide funding for outings and athletic events for Hook graduates and community members with disabilities (Adaptive Athletics Program)

Hook Pet Therapy
To support care of the network's therapy dogs for patient assistance in Hook Rehab Center

Hook Rehabilitation Center
To provide training and development for staff of Hook Rehabilitation

Hospice Fund
Revenue from memorials and program support will assist program for hospice program patients and families

Life's Journey: Choices in Care
The Life’s Journey: Choices in Care program spans the network continuum of care, from inpatient to home care to physician office to home in the community and to hospice. The fund may be used to enhance the quality of patient care, improve staff competency, expand the program within Community's continuum of care, and to expand our knowledge on chronic disease management and palliative care.

Madonna Trout Hook Rehab Education
To support education and training for Hook Rehabilitation speech pathologists

Maternal-Fetal Medicine
Support maternal-fetal medicine services/sponsored events

Medical Education
To support medical education of physician leadership across the network

Neonatal Intensive Care (NICU)
To provide items for NICU staff and patients that do not come from capital money, yet add value to clinical care/outcomes (education, supplies, minor equipment)

Neuro-Oncology Research
Support Community Regional Cancer Center neuro-oncology clinic programs and patient needs

Neuroscience Services
Neuroscience services educational conference (materials), food (celebrations), minor equipment, special needs/requests

Neuroscience Nursing Scholarship
To support neuroscience programs of Community

Nursing Operations Fund
To support nursing excellence operations for Community Health Network

Open Arms Bereavement Program
To provide support and care to families who have experienced a miscarriage, stillbirth, or newborn death within maternity services, including NICU. Support will include memorial services, burial services for miscarriages and follow-up care for families. To provide training to hospital and community care givers in the city and surrounding counties. Support of perinatal hospice program. (Open Arms)

Orthopedic Services
Funds to be utilized to support inpatient and outpatient orthopedic service line (i.e., technology, equipment, and other program needs)

Patient Comfort and Amenity Fund
Provides clothing and other necessities to patients in need

Parish Nursing
Operating costs for parish nursing and health ministries

Pharmacy Fund
Education, research and technology that supports the mission of pharmacy in support of the network's mission and vision

Physical Therapy
Equipment, etc., for clinic (Rehab & Sports Medicine Centers)

Pulmonary Rehab Services
Equipment needs/Educational material

Radiology Equipment
To purchase equipment to support radiology services in Community Hospitals East, North and South not covered in annual capital allocations

Rehab/Brain Injury
Support innovations in brain injury services

Residency Activity Fund
Resident wellness and facility improvement

Respiratory Therapy
Education conferences (materials), patient celebration (food items), and minor equipment

Serve 360 Fund
Supports the Community Health Network activities of the Serve 360 program

Stephanie Isenberg Scholarship Fund
Supports nursing scholarships within Community Health Network

Surgical Services
Contributions restricted for use by surgical services of Community Hospitals Indianapolis

Touchpoint Program
To support the Touchpoint program older adult initiatives throughout the Indianapolis community

Women's & Children's Services
To support services to patients and families to assure an exceptional patient/family experience (women's services | children's services)

Wound, Ostomy & Incontinence
To support continuing medical education for WOC nurses throughout the network

Permanently restricted funds

Anita J. Harden Endowment to Benefit the Seriously and Persistently Mentally Ill
To distribute, as needed, funds for patient activities, programs and services not normally covered by insurance or publicly funded sources that will improve the quality of life of adults (including transition-aged youth) living in Central Indiana who have been diagnosed and are living with a serious and persistent mental illness.

Betty Brown Memorial Fund
Patient care concerns of Psychology Department in Oncology unit

General operations of CHHS

Health Careers Scholarship Endowment
To support scholarships

Henry Olivier Endowment
To support education related to cardiovascular disease

Provide scholarships for employees of CHNw to continue educational opportunities in the field of healthcare
