Life's Journey: Choices in Care
The Life’s Journey: Choices in Care program spans the network continuum of care, from inpatient to home care to physician office to home in the community and to hospice. The fund may be used to enhance the quality of patient care, improve staff competency, expand the program within Community's continuum of care, and to expand our knowledge on chronic disease management and palliative care.

To provide support and care to families who have experienced a miscarriage, stillbirth, or newborn death within maternity services, including NICU. To provide training to hospital and community care givers in the city and surrounding counties. Support of perinatal hospice program. (Open Arms)
