Community Health Network Foundation awarded $2.4 million in federal grants

Community Health Network Foundation, which raises financial support for Community Health Network, has been awarded more than $2.4 million in grants from the U.S. Department of Health and Human Services’ Health Resources and Services Administration (HRSA). The three grants will be used to transform Community’s current Family Medicine Residency Program (FMRP) into a Patient-Centered Medical Home (PCMH) model, increase resident enrollment and purchase medical training equipment. The PCMH model is a family physician-led approach to providing comprehensive primary care for people of all ages and medical conditions through patient care coordination. Full story >>

Funding clinical excellence

Contributions from eastside Indianapolis residents built Community Hospital East, an innovative center for clinical excellence, high-quality patient outcomes and compassionate care that opened in 1958. Since then, donations continue to fund the capital budgets needed to construct an innovative health care network that has evolved to four Indianapolis hospitals and more than 70 central Indiana sites of care.

Our donors are making possible the research and delivery of innovative health care. Their financial support helps us purchase the best technology and equipment needed to treat patients shaken with illness, disease or emergency. And, their generosity has helped build state-of-the art health care facilities at Community Hospital East, Community Hospital North, The Indiana Heart Hospital and Community Hospital South where innovation and compassion have spurred the next evolution of health care.

But contributions do not just support innovation at our four hospital locations and the numerous health services and programs you will find there. They also allow us to support the network of clinics, physician offices, health pavilions and other neighborhood-focused sites of care that bring innovative, quality care close to home.
